Perimenopause and an itchy vulva: what to know

Short answer
Perimenopause can cause vulval dryness and itching, but infection, irritation, or a skin condition may need different treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Avoid
- Scented washes, douches, wipes, and deodorants on the vulva.
- Arrange review
- Persistent itching or itching with sores, bleeding, discharge, or pain.
Possible reasons for itching
Changing oestrogen levels can make vulval skin and nearby tissues drier and more sensitive. Soaps, fragranced products, pads, sweat, tight clothing, thrush, bacterial imbalance, sexually transmitted infections, eczema, and other skin conditions can also cause itching. Burning, soreness, discharge, odour, cracks, or pain with sex provide useful clues but do not identify the cause by themselves.
Gentle care while you arrange help
Wash the vulva with lukewarm water or a bland, fragrance-free cleanser, then pat dry. Avoid douching, scented wipes, deodorants, and scratching; wear breathable underwear and change out of damp clothing. Do not repeatedly use an over-the-counter thrush treatment when the diagnosis is uncertain. A clinician can examine the skin and test discharge if needed.
When to seek care
Arrange an appointment for itching that persists, returns, affects sleep, or comes with discharge, odour, sores, bleeding, skin colour or texture changes, or pain. Seek prompt care for severe swelling, fever, pelvic pain, or difficulty passing urine. Questions for your doctor
Could this be dryness, thrush, an infection, eczema, or another skin condition? Should the skin or discharge be tested? Which moisturiser or local treatment is suitable, and what products should I avoid?

Questions people ask
Is vulval itching always due to perimenopause?
No. Dryness can contribute, but infections, contact irritation, eczema, and other skin conditions are also possible.
Should I treat suspected thrush myself?
If the diagnosis is uncertain, repeated self-treatment can delay the right care. Assessment is useful when symptoms are new, persistent, or unusual for you.
