Perimenopause after hysterectomy: what to know

A hysterectomy removes the uterus, and some operations also remove the cervix or ovaries. If the ovaries remain, they can continue producing hormones and later become less predictable during Perimenopause. Without periods, changes such as Hot Flushes, Night Sweats, sleep disruption, mood changes, vaginal symptoms, or Low Sex Drive may be the main clues. If both ovaries were removed, the hormone change can begin suddenly rather than follow a gradual transition. The operation type and your age affect how symptoms are interpreted.

Ask for your operation record if you are unsure whether your ovaries or cervix remain, and note when symptoms began. Tell your clinician about the surgery, cancer history, medicines, migraine history, clotting history, and any symptoms such as Missed Period or Irregular Periods that occurred before surgery. They can discuss symptom relief, bone and cardiovascular health, vaginal comfort, and whether tests are useful; hormone results may not provide a simple answer on their own.

Seek urgent help for chest pain, sudden breathlessness, one-sided leg swelling, severe headache with neurological symptoms, or heavy bleeding. After hysterectomy, any vaginal bleeding should be discussed with a clinician, especially if it is new, recurrent, or accompanied by pelvic pain, fever, or unusual discharge.
