Peeing when you cough during pregnancy

A cough briefly increases pressure inside your abdomen. As the uterus grows, the bladder has less room and the pelvic floor muscles may have more work to do, so a small amount of urine can escape. Leakage may be more noticeable when your bladder is full, when you laugh, sneeze, lift something or stand up quickly.

This symptom does not by itself show that you have a urinary infection. Burning, an urgent need to pass urine, lower abdominal discomfort, fever or blood in the urine point to a different problem that needs assessment. Persistent cough also deserves attention; causes can include a cold, allergies, reflux or asthma.

Empty your bladder regularly, avoid waiting until it is uncomfortably full, and drink enough fluid so your urine is not consistently concentrated. Do not restrict fluids to control leakage. Before coughing, gently tighten the muscles used to stop passing wind, then release after the cough; a pelvic-floor physiotherapist or maternity clinician can check your technique.

Treat the cough safely in pregnancy with advice from your maternity team or pharmacist, especially before taking a cough or cold product. Keep a brief record of triggers, leakage and urinary symptoms so your clinician can identify patterns.

Contact your maternity team promptly for painful urination, fever, back or side pain, blood in the urine, worsening leakage, or a cough with breathing difficulty or chest pain. Seek urgent help if you cannot catch your breath, feel faint, or cough up blood.

Ask whether your symptoms fit stress leakage, whether you need a urine test, and which pelvic-floor exercises are suitable at your stage of pregnancy. Ask which cough treatments are appropriate for you and when ongoing leakage should be reviewed.
