What it may mean

Intense menstrual pain can trigger a reflex that briefly lowers blood pressure and heart rate, causing light-headedness or fainting. Dehydration, not eating, anaemia from heavy periods, and standing suddenly can add to the risk. Conditions such as adenomyosis, endometriosis, ovarian endometrioma, pelvic inflammatory disease, or uterine fibroids can cause severe or worsening period pain.

Fainting is different from ordinary cramps. A clinician may ask about the timing of the episode, bleeding, pregnancy possibility, medications, and symptoms such as fever or unusual discharge.

What to do now

If you feel faint, lie flat with your legs raised, loosen tight clothing, and move away from heat. Do not drive, climb, or stay alone until you feel fully recovered. Sip fluids if you are awake and able to swallow. Arrange a prompt appointment, especially if this has happened before or your periods have become more painful or heavier.

Keep a brief record of pain, bleeding, dizziness, medicines, and recovery. A clinician can discuss pain relief and options such as the Combined Pill or Hormonal Coil when appropriate.

When to seek urgent care

Seek emergency help if fainting is followed by chest pain, trouble breathing, confusion, a severe headache, injury, ongoing weakness, or difficulty waking. Get urgent assessment for very heavy bleeding, severe one-sided pelvic pain, fever, or possible pregnancy. Do not assume the episode is only period pain.
