Paroxysmal cough differential diagnosis

Clinicians look at onset, duration, frequency, exposures, and what happens before and after a bout. A cough after a cold may reflect lingering airway irritation or acute bronchitis. Night cough, wheeze, chest tightness, or exercise-related symptoms can point toward asthma. Sour taste, hoarseness, or symptoms after meals can suggest acid reflux, while a blocked or dripping nose may indicate allergic rhinitis.

Mucus, repeated chest infections, breathlessness, smoking or workplace exposures, and weight or appetite changes add useful context. The same symptom can have more than one contributor, so the assessment is built from the whole pattern rather than a single clue.

Keep a short cough diary: record timing, triggers, sleep disruption, mucus, fever, wheeze, breathlessness, reflux symptoms, and medicines. Bring details of recent infections, allergies, smoking or vaping, dust or chemical exposure, and any inhalers. Do not start antibiotics or change prescribed inhalers without advice.

Depending on the history, a clinician may examine your nose and chest, check oxygen levels, arrange breathing tests, or consider imaging. Testing is chosen to answer a specific question and may change as the symptoms evolve.

Seek emergency help for severe breathing difficulty, blue or grey lips, chest pain, fainting, confusion, coughing blood, or sudden inability to speak normally. Arrange prompt review when coughing bouts are increasing, follow an injury or choking event, come with high fever, or prevent normal sleep and activity.

Ask which causes fit your timing and triggers, what examination or test would distinguish them, and what result would change treatment. Clarify what to do during a bout and when a planned follow-up should become urgent.
