How pallor can develop

Skin may become pale when small blood vessels narrow, such as during cold or fear, or when blood flow is reduced. It may also look pale when the blood has fewer red cells or less haemoglobin available to carry oxygen. Iron deficiency, vitamin B12 deficiency, thalassaemia, and sickle cell disease can present with pallor, but the symptom does not show which condition is responsible. A naturally light complexion is different from a new loss of colour.

How to investigate it

Check whether the change is sudden or gradual, general or limited to one area, and constant or intermittent. Look for tiredness, dizziness, headaches, breathlessness, palpitations, cold hands, fever, pain, jaundice, or bleeding. Review recent illness, food intake, heavy menstrual bleeding, digestive symptoms, and medicines with a clinician. They may examine the eyes, mouth, skin, and circulation and request blood tests. Avoid taking supplements before the likely cause is clear.

When it needs urgent attention

Call emergency services for sudden pallor with collapse, confusion, severe breathlessness, chest pain, blue lips, cold clammy skin, or heavy bleeding. Arrange medical review when the change has no clear short-lived trigger, keeps returning, or affects daily activity. Seek earlier advice for a child, pregnancy, significant chronic illness, or inherited blood condition.

Ask which symptoms make anaemia or another cause more likely, whether you need a blood count and nutrient tests, and what treatment would depend on the result. Bring a list of medicines, supplements, dietary restrictions, and any family history of blood disorders.
