Why skin may look blue
Blue or grey colouring can be harder to judge under artificial light and on different skin tones. Check the lips, tongue, gums and nail beds, and compare with the person’s usual appearance. Cold can narrow blood vessels and make fingers look pale or bluish. A problem with breathing, circulation or oxygen delivery can also cause blue colouring, especially around the lips or face. Anaemia, iron deficiency anaemia, vitamin B12 deficiency, Sickle Cell Disease and Thalassaemia affect blood in different ways, but a blue appearance does not diagnose any of them.

Immediate steps
If the person is cold but otherwise well, move them indoors, remove wet clothing and warm them gradually. Do not use very hot water or direct intense heat on numb skin. Keep watching breathing and responsiveness. If a prescribed rescue medicine or oxygen plan exists, follow it as directed. Do not give food or drink to someone who is drowsy or struggling to breathe. Arrange assessment if blue colouring persists after warming or keeps returning.
When it is an emergency
Call emergency services immediately for blue lips or face, severe breathlessness, gasping, chest pain, collapse, confusion, inability to wake, or a rapidly worsening colour change. Do not drive someone who is severely breathless or confused. If an infant, child, pregnant person or person with heart or lung disease develops blue colouring, seek urgent help even if the change appears brief.

Ask what caused the colour change, whether oxygen, heart or lung checks are needed, and how to recognise recurrence. If a blood condition is suspected, ask which tests confirm it and whether family history is relevant.
