Painful red spots on hands and feet: what they may mean

The appearance matters: small raised bumps around hairs can fit folliculitis, while firm tender spots without a visible hair may reflect friction, contact irritation, or another skin problem. Pimples are less typical on palms and soles because those areas do not have the same hair follicles as much of the surrounding skin. A clinician may ask whether the spots itch, blister, scale, spread, or follow a new product, glove, shoe, or activity.

Wash gently with lukewarm water, pat dry, and avoid picking, squeezing, harsh scrubs, and fragranced products. Reduce rubbing from tight footwear or gloves and keep the area cool and dry. Do not apply leftover antibiotics or steroid creams without advice, especially if the spots are blistering or broken. Take a clear photograph if the appearance changes, and arrange a dermatology assessment if the spots persist, recur, or interfere with walking or using your hands.

Seek prompt medical care if redness is rapidly spreading, the skin is hot and swollen, pus is present, pain is severe, or you feel feverish or unwell. Urgent assessment is also sensible for purple or bruise-like spots that do not fade with pressure, new blisters affecting the mouth or eyes, or swelling that makes movement difficult.

Ask whether the pattern fits acne, folliculitis, contact irritation, or another condition; whether a swab or other examination is needed; which products are safe for hands and feet; and what change should trigger an earlier review.
