What the pain can mean

Mittelschmerz means mid-cycle pain. It may feel like a twinge, cramp, pressure, or sharper discomfort on the side where an ovary is active. The sensation can last briefly or continue for part of the day, and the side may change from one cycle to another. Some people notice light spotting or a small amount of vaginal discharge at the same time.

Ovulation pain is not the same as menstrual pain, and a familiar pattern can make it easier to recognise. Pain outside your usual pattern can have another explanation. Endometriosis can present with pelvic pain, while ovarian cysts can also cause discomfort; neither condition can be diagnosed from timing alone.

Ways to manage it

Record when the pain starts, which side hurts, how long it lasts, and whether bleeding, discharge, nausea, or fever occurs. Rest, gentle movement, warmth over the lower abdomen, and fluids may make mild discomfort easier to tolerate. A pharmacist or clinician can help you choose an appropriate pain reliever, especially if you may be pregnant, have stomach or kidney problems, or take other medicines.

Arrange a routine appointment if episodes are becoming stronger, last longer, interfere with daily activities, or occur at times unrelated to ovulation. A clinician may ask about your cycle and examine you before deciding whether further assessment is useful.

When to seek care

Seek urgent medical care for sudden severe pelvic pain, fainting, marked dizziness, heavy bleeding, fever, repeated vomiting, a swollen abdomen, or pain during a possible pregnancy. These warning signs need prompt assessment because ovulation timing does not explain every cause of pelvic pain.
