Pain in the lower right abdomen around ovulation

Right-sided lower abdominal pain around ovulation may be mid-cycle pain, but similar symptoms can have other causes. The timing, duration, severity, and associated symptoms help determine whether it needs medical assessment.

Ovulation pain, also called mittelschmerz, can occur when an ovary releases an egg. It may be felt on one side and may recur around the middle of a cycle, but a right-sided symptom does not confirm ovulation. An ovarian cyst can cause one-sided pain, pressure, or pain that changes suddenly. Endometriosis may cause recurring pelvic pain, painful periods, pain with sex, or bowel and bladder symptoms.

A clinician may ask about your last period, cycle regularity, pregnancy possibility, bowel and urinary symptoms, and whether the pain is new or different from previous cycles.

Record when the pain starts, how long it lasts, and where it sits in relation to your cycle. Rest, gentle warmth, and ordinary pain relief may help some people if these options are suitable for them; follow the product directions and check with a pharmacist if you have a health condition or take other medicines. Arrange an assessment if the pain keeps returning, is becoming stronger, or does not follow your usual pattern. Testing may include a pregnancy test or ultrasound when indicated.

Seek urgent care for sudden severe right-sided pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, or pain with a possible pregnancy. Prompt review is also appropriate when pain persists, makes movement difficult, or comes with marked abdominal tenderness, unusual discharge, or urinary symptoms.
