Left-sided pain around ovulation: what to know

Ovulation pain may feel like a brief cramp, twinge or ache on the side releasing an egg and can occur around the middle of a cycle. It may be accompanied by a small amount of spotting or vaginal discharge. Endometriosis can cause recurring pelvic pain that follows a cycle and may also cause painful periods, pain during sex or bowel symptoms. An ovarian cyst can produce one-sided pressure or pain and may become suddenly severe if it bleeds or twists. Digestive and urinary problems can mimic pelvic pain.

Record the date, side, intensity and duration of each episode alongside your menstrual cycle. Rest, gentle heat and a pain-relief option suitable for you may help; ask a pharmacist if pregnancy is possible or you take regular medicines. Seek a gynaecology assessment when episodes are new, recurrent, worsening or disruptive, or when pain comes with bleeding, discharge, urinary or bowel changes.

Seek urgent care for sudden intense one-sided pain, faintness, heavy bleeding, fever, vomiting, shoulder-tip pain or a positive pregnancy test with abdominal pain. Prompt assessment matters because these features can occur with an ovarian emergency, infection or pregnancy-related complication. Do not wait for the next cycle if the pain is severe or rapidly worsening.
