Pain During Sex and Fibroids

Fibroids can make sex painful when their size or position creates pressure on the uterus, cervix or surrounding pelvic tissues. Some people notice deep aching during penetration, pelvic heaviness or discomfort in particular positions. Fibroids are not the only explanation for pain, so the symptom needs to be considered alongside bleeding, cycle changes and other pelvic symptoms.

Fibroids are non-cancerous growths of uterine muscle. A fibroid that projects toward the uterine cavity may be associated with heavy or prolonged periods, while one on the outer surface can create pressure in the pelvis. Endometriosis, ovarian endometrioma, infection and pelvic-floor tension can produce similar deep pain. Endometriosis commonly presents with pain during sex, particularly when pain is cyclical or accompanied by painful periods.

A clinician may use your history, pelvic examination and ultrasound to assess whether fibroids are present and whether their location could explain the discomfort.

Use positions with less depth, move slowly and stop when pain starts. Record bleeding pattern, pressure, urinary or bowel symptoms and the relationship to your period. Arrange a gynaecology appointment if pain repeats or if you have heavy bleeding, anaemia symptoms or pelvic pressure. Options depend on symptoms, fibroid location, size, pregnancy plans and your preferences; observation, medicines, procedures or surgery may be discussed.

Get urgent help for sudden severe pelvic pain, faintness, heavy bleeding, fever, repeated vomiting or possible pregnancy with pain. Seek a prompt appointment for new bleeding after sex, rapidly worsening pressure, persistent pain or fatigue and breathlessness that may accompany significant blood loss.
