Pain During Sex Before Your Period: What to Know

The timing may be a useful clue, but it does not identify one diagnosis by itself. Note whether the pain is at the vaginal opening or deeper in the pelvis, whether it happens with every cycle, and whether bleeding, discharge, fever, or pain outside sex occurs too.

Before menstruation, pelvic tissues can feel more tender and cramping may begin. Deep pain that repeatedly appears in the days before bleeding can occur with endometriosis, an ovarian endometrioma, or pelvic inflammatory disease. A breast or pelvic symptom can have many benign explanations, so this pattern should prompt assessment rather than a conclusion.

Pain at the entrance may be related to dryness, skin irritation, involuntary pelvic-floor tightening, or vulvodynia. Genitourinary Syndrome of Menopause and Lichen Sclerosus can also cause dryness, burning, soreness, or fragile skin, particularly around the vaginal opening.

A clinician will usually ask about your cycle, contraception, pregnancy possibility, infections, previous pelvic procedures, and the exact location and character of the pain. An examination and selected tests can help separate surface irritation from deeper pelvic causes.

Pause penetration when it hurts and choose activities that do not cause pain. More time for arousal and a plain, fragrance-free lubricant may help if dryness or friction is contributing. Avoid scented washes, douching, and products that sting the vulva.

Keep a brief symptom diary for several cycles: record the first day of bleeding, when pain starts, its location, depth, severity, associated discharge or bleeding, and whether bowel or bladder symptoms occur. Arrange a gynaecology appointment if the pattern recurs, is worsening, or interferes with intimacy. Treatment depends on the cause; options may include treating an infection or skin condition, pelvic-floor therapy, hormonal treatment, or further evaluation for endometriosis.

Seek urgent medical care for severe or rapidly worsening pelvic pain, fainting, heavy bleeding, fever, repeated vomiting, or pain with a possible pregnancy. These symptoms can need prompt assessment, including for infection or an ectopic pregnancy.

Book a timely appointment for new pain, pain after sex with bleeding, unusual or foul-smelling discharge, sores or marked itching, pain when urinating, or pain that continues after your period. Do not wait for the next cycle if symptoms are strong or unusual for you.
