How clinicians think about it

Pain at the vaginal opening with a small smear of blood can follow friction, a skin condition, or pelvic-floor muscle tightening. Deep pain with bleeding may involve the cervix, uterus, ovaries, or inflamed pelvic tissues. Genitourinary syndrome of menopause can cause dryness and fragile vaginal tissue. Endometriosis and an ovarian endometrioma can cause deep, cycle-linked pain, while pelvic inflammatory disease can produce pelvic pain, discharge, fever, or pain during an examination. Vulvodynia may cause burning or tenderness at the entrance.

Bleeding after sex is not enough to identify one cause. A clinician will ask about pregnancy possibility, menstrual timing, contraception, infection exposure, menopause symptoms, screening history, and whether bleeding happens at other times.

Steps before an appointment

Stop penetration if it hurts or bleeding starts. Avoid fragranced washes and use a lubricant that feels comfortable if you resume sex after symptoms settle. Write down the amount and colour of blood, pain location, duration, cycle day, discharge, fever, urinary symptoms, and any new partner or contraceptive change. Book a medical review when the symptom is new, repeated, or unexplained; testing may involve examination, infection tests, cervical assessment, or imaging.

Do not delay urgent care

Seek urgent help for severe or increasing pain, heavy bleeding, faintness, fever, repeated vomiting, or a positive pregnancy test with pain or bleeding. Prompt review is also needed for persistent bleeding, bleeding after menopause, or a new lump or unusual discharge.

Does the bleeding appear to come from the vaginal tissue or cervix? Should I have infection testing, cervical screening, or an ultrasound? Could pelvic-floor tension, endometriosis, or dryness contribute, and what treatment can protect comfort while the cause is clarified?
