Ovulation pain when trying to conceive

Sperm can remain available for fertilisation before an egg is released, so waiting until pain begins may not capture the whole fertile window. Ovulation pain may be a one-sided ache, cramp, or brief sharp sensation, with changes in cervical mucus occurring around the same phase. Cycles vary, and some people ovulate without feeling pain.

Pain does not show whether the fallopian tubes, ovaries, uterus, or sperm are functioning normally. Endometriosis may affect pelvic health and fertility, and an ovarian cyst may cause pain; neither can be diagnosed from ovulation pain alone.

Track cycle dates, cervical mucus changes, and the timing, side, and duration of pain. Consider discussing an ovulation predictor test with a pharmacist or clinician, especially if cycles are irregular. When trying to conceive, regular intercourse across the fertile window may be more useful than relying on one painful moment. Review medicines before pregnancy, including pain relievers and supplements.

Arrange a fertility or gynaecology review for repeated severe pain, pain during sex, very irregular periods, bleeding between periods, or difficulty conceiving. Seek urgent care for severe sudden pain, fainting, heavy bleeding, fever, persistent vomiting, or pelvic pain with a positive pregnancy test.
