When ovulation cramps feel worse than period cramps

Short answer
Ovulation cramps can feel worse than period cramps, yet severe or unusual pain should be assessed because several pelvic conditions can cause it.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Timing, duration, side and associated symptoms
What the difference may mean
Mid-cycle pain may come from the ovary releasing an egg, irritation from fluid, or a brief muscle spasm. It can be one-sided, sharp or aching and may last from a short time into a couple of days. Pain that repeatedly disrupts daily activities, occurs with painful periods, or gradually becomes stronger can point to another cause rather than ordinary ovulation pain. Endometriosis, adenomyosis, fibroids, pelvic inflammatory disease and an ovarian endometrioma are examples a clinician may consider. A painful period pattern can also be useful context; see Painful Periods for related information.
What you can do now
Record when the pain starts, which side it affects, how long it lasts, and whether bleeding, discharge, bowel symptoms or urinary symptoms occur. Rest, gentle movement, warmth over the lower abdomen and fluids may help. Use a pain medicine only if it is suitable for you and follow its package directions; ask a pharmacist if pregnancy, ulcers, kidney disease, allergies or other medicines make the choice uncertain. If episodes recur, arrange a routine appointment to discuss examination, pregnancy testing where relevant, and whether an ultrasound is appropriate. Hormonal options such as the Combined Pill or Hormonal Coil may be discussed when cycle-related pain is persistent.
When to seek care
Seek urgent medical care for sudden severe pain, fainting, marked dizziness, heavy bleeding, fever, repeated vomiting, a swollen abdomen, or pain with a possible pregnancy. Prompt assessment is also sensible when pain is new, steadily worsening, follows sex, or comes with unusual vaginal discharge. Do not rely on the timing of the cycle to explain a red-flag symptom.
Questions to take to an appointment
Could this pattern fit ovulation pain, or should we check for another pelvic cause? Would an examination, pregnancy test or ultrasound help? Which pain relief is appropriate for me, and what symptoms should prompt urgent review?

Questions people ask
Can ovulation pain be sharper than period pain?
Yes. It may feel sharp or sudden, but intense, persistent or unfamiliar pain should be assessed.
Does severe ovulation pain diagnose endometriosis?
No. Endometriosis is one possible explanation among several, so assessment depends on the full symptom pattern.
