Over-the-counter care for perioral dermatitis

Perioral dermatitis is a rash of small red or skin-coloured bumps around the mouth, nose, or eyes. The skin may burn, itch, flake, or feel sensitive, and a narrow area next to the lips may remain clear. It can resemble pimples, but acne products are not automatically suitable.

Topical corticosteroids may briefly reduce redness while allowing the condition to continue or rebound after stopping. Other triggers can include heavy occlusive products, fragranced cosmetics, or irritation from a complex routine. The exact trigger is not always obvious.

Use lukewarm water and a mild, fragrance-free cleanser if needed. Pause scrubs, peels, fragranced products, heavy face creams, and non-essential actives until the skin settles. Choose a simple moisturiser if tightness is uncomfortable, and apply products carefully around the mouth and nose.

Do not start or restart a steroid cream on the rash without medical direction. If symptoms began after a prescribed steroid, contact the prescriber for a safe plan rather than stopping or changing it in isolation. Persistent cases may need topical antibiotic therapy or oral antibiotics prescribed for the individual situation.

Arrange a review if the rash is spreading, recurring, affecting the eyes, or not improving after a simplified routine. Seek prompt help for eye pain, vision change, facial swelling, pus, fever, or rapidly worsening redness. Assessment is useful when the bumps do not fit the usual pattern or treatment has made them worse.

Ask whether the rash is perioral dermatitis or another condition, whether a steroid needs tapering or replacement, which cleanser and moisturiser to use, and whether prescription topical or oral treatment is appropriate.
