What should you know about outer hamstring strain?

Short answer
Outer hamstring strain can cause sudden or gradual pain at the back and outer thigh, with weakness during running or bending the knee, and recovery depends on a graded return to loading.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical area
- Back and outer thigh, sometimes near the buttock or back of the knee
- Common triggers
- Sprint starts, kicking, stretching, or sudden direction changes
- Recovery focus
- Progressive strength, speed, control, and sport-specific loading
What the injury involves
The hamstrings cross the back of the hip and knee. A strain can affect muscle fibres or the tendon, often during sprinting, stretching, kicking, or a sudden change of direction. The outer hamstring may feel sore near the thigh, buttock, or back of the knee. Bruising, swelling, a pulling sensation, reduced stride length, and pain when bending the knee against resistance may occur. Thigh Pain has several possible causes, including nerve, hip, or back problems, so the injury mechanism and examination help distinguish them.
Supporting recovery
In the early phase, stop sprinting and movements that cause a sharp pull. Walk only as comfortably as possible, and use a wrapped cold pack if it helps with pain. Avoid aggressive stretching or massage over a freshly injured area. Once daily movement is comfortable, rehabilitation usually progresses through gentle contractions, controlled strength work, faster movements, and sport-specific drills. Exercise Therapy, Sports Physiotherapy, and a Return-to-Sport Programme can help match loading to strength and control. PRP Injection may be discussed in selected cases, but it is not a substitute for diagnosis and rehabilitation.
When to seek care
Arrange assessment if pain causes a limp, limits normal walking, returns when you increase activity, or does not steadily improve. Prompt review is appropriate after a popping sensation, extensive bruising, a visible gap, or marked weakness. Seek urgent help if the leg becomes very swollen, numb, unusually cold, or painful at rest, or if breathlessness or chest pain occurs with new leg swelling.
Questions for your doctor
Which part of the hamstring is injured, and how severe does it appear? When can I start strengthening and running? Which tests should I pass before returning to sport? Could the back or hip be contributing? Would imaging change my rehabilitation plan?

Questions people ask
Should I stretch an outer hamstring strain?
Avoid forceful stretching early on. Gentle movement can be introduced as pain settles, followed by guided strengthening and gradual lengthening work.
When can I return to sport?
Return when walking, strength work, faster running, and sport-specific movements are comfortable without a symptom flare afterward; a clinician can test readiness.
