Orthostatic hypertension signs and symptoms

The defining clue is a repeatable blood-pressure rise when moving upright, not a single feeling. Some people notice a pounding heartbeat, head pressure, warmth, lightheadedness, blurred vision, or unsteadiness; others feel nothing. Similar symptoms can occur with ordinary hypertension, medication effects, dehydration, anxiety, or heart and neurological conditions, so they do not establish the cause by themselves.

A clinician may compare readings in different positions and review your pulse, medicines, sleep, kidney health, weight, and cardiovascular history. Write down whether symptoms follow standing, meals, exertion, heat, or a dose of medicine, and whether sitting improves them.

If symptoms start, sit or lie down somewhere safe and wait until your balance and vision settle. Rise gradually, keep a stable support nearby, and avoid driving or climbing while affected. If home monitoring has been recommended, follow the same rest period, cuff position, and posture each time so readings can be compared.

Arrange a review for recurring symptoms, repeated high readings, falls, or a new pattern after starting or changing treatment. Bring your readings and a complete medicine list. Do not stop treatment or make major changes to salt, fluids, or exercise without advice tailored to your health.

Get emergency help for chest pain, severe breathlessness, fainting, sudden weakness or numbness, facial drooping, difficulty speaking, confusion, a sudden severe headache, or sudden vision loss. These warning signs can accompany stroke, angina, or another serious problem. A symptom-free reading still warrants routine follow-up when the rise is repeated.

Ask how to measure blood pressure after standing, how often to record it, and whether another monitoring method is useful. Discuss possible contributions from hypertension, sleep apnoea, kidney disease, weight, or medicines, plus the specific symptoms that should prompt emergency care.
