What should you know about optical seizure?

Seizures are bursts of abnormal electrical activity in the brain. Some begin in areas involved in vision, causing flashing lights, coloured shapes, blurred vision, narrowed vision, or a strong sense that an image looks unfamiliar. A person may remain aware, or awareness may become impaired as the seizure spreads. Visual symptoms can also have other explanations, so a first episode should be assessed rather than self-diagnosed.

Possible triggers or contributing problems include low blood sugar, alcohol withdrawal, infection, head injury, or an underlying neurological condition. A seizure is not contagious, and witnessing one does not mean you caused it.

Guide the person away from traffic, stairs, water, and sharp objects. Stay with them, loosen tight clothing around the neck, and note when the episode starts and what the eyes and body do. Do not restrain them, put anything in their mouth, or give food, drink, or tablets until they are fully alert. When movements stop, place them on their side and speak quietly as they recover.

Arrange medical assessment after a first suspected seizure, even if the person feels well afterwards. Tell the clinician about sleep loss, medicines, alcohol use, illness, head injury, and any warning vision changes.

Call emergency services if the seizure lasts longer than usual, repeats without recovery, causes serious injury, happens in water, or the person has trouble breathing. Urgent help is also needed for a first seizure, pregnancy, diabetes, severe headache, fever, stiff neck, new weakness, or ongoing confusion. Sudden persistent loss of vision is an emergency even without shaking.

Ask what tests may clarify the cause, whether driving or swimming precautions are needed, how to record future episodes, and which symptoms should prompt emergency help. Ask whether a medication, sleep pattern, alcohol change, or blood sugar problem could have contributed.
