One Small Bald Spot on the Scalp: What to Know

A smooth, clearly defined patch without scale may occur with alopecia areata. Scalp ringworm can produce hair loss with scale, broken hairs, itch, tenderness, or swollen areas and needs medical treatment. Tight hairstyles or repeated pulling can also create a local patch. A scarred, shiny, painful, or inflamed area raises a different concern because damaged follicles may not regrow hair. A clinician can inspect the scalp and hairs rather than relying on the patch alone.

Photograph the patch in consistent lighting and note its size, border, symptoms, and any new spots. Avoid tight styles, scratching, sharing combs, hats, or pillows, particularly when scale or broken hairs suggest infection. Do not use steroid or antifungal treatment on the scalp without advice, because the wrong product can obscure the examination or delay appropriate care. Arrange a dermatology review if the cause is uncertain.

Seek prompt care for a painful or swollen scalp, pus, marked scale, fever, rapidly enlarging loss, or several new patches. Children with a scaly patch or broken hairs should be assessed because scalp infection can spread between close contacts. Earlier review is also sensible when the area looks shiny or scarred, or when eyebrows or body hair are changing.

Ask whether the patch looks smooth or scarred, whether testing is needed for infection, and which treatment fits the diagnosis. Discuss how to monitor regrowth and whether a hair transplant would be unsuitable while active hair loss or inflammation remains.
