Short answer

One swollen ankle can follow injury, inflammation, infection, a vein problem, or fluid buildup and needs prompt assessment when sudden or painful.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Key detail
Sudden onset or swelling without an injury
Avoid
Massaging a warm, painful or suddenly swollen leg
Emergency symptoms
Chest pain or sudden breathlessness

Possible explanations

A twist, strain, bruise or fracture may cause swelling with pain and difficulty bearing weight. Warmth, redness and tenderness can indicate inflammation or infection. A clot in a deep vein can cause one-sided swelling, aching or tightness, especially when it begins suddenly. Varicose Veins can contribute to pooling and heaviness. Conditions such as Heart Failure or Chronic Kidney Disease more often cause swelling in both legs, but a new one-sided change still needs its own assessment. Obesity can increase strain on joints and veins without identifying the cause by itself.

What to do now

If the swelling followed a minor injury, rest the ankle, raise it when comfortable and use a wrapped cold pack for short periods. Avoid tight footwear and do not massage a suddenly swollen, warm or painful leg. Note when it started, whether the calf is affected, and any redness, fever, chest symptoms or breathlessness. Arrange medical advice promptly for unexplained one-sided swelling, even if the discomfort is mild. A clinician may examine the ankle and leg and decide whether imaging or other tests are needed.

When to seek care

Seek urgent help if ankle or leg swelling comes with chest pain, sudden breathlessness, coughing blood, faintness, or a fast heartbeat. Get same-day assessment for sudden swelling, significant pain, warmth, redness, fever, blue or pale skin, numbness, or inability to walk. After an injury, urgent review is also appropriate if the ankle looks deformed or pain is severe.

Questions for your doctor

Could this be an injury, infection or a vein problem? Do I need an ultrasound or an X-ray? Should I change activity, footwear or any medicine while the cause is being assessed?
