What should you know about occult ectopic pregnancy?

An ectopic pregnancy implants outside the main cavity of the uterus, most often in a fallopian tube. The word occult means that the pregnancy has not yet been clearly identified, which can happen when the pregnancy is very early, the scan cannot locate it, or the findings do not yet provide a definite diagnosis. A positive pregnancy test with no pregnancy visible in the uterus does not by itself confirm an ectopic pregnancy; it needs careful clinical follow-up.

Clinicians may consider factors such as one-sided pelvic pain, vaginal bleeding, previous ectopic pregnancy, or conditions that affect the fallopian tubes. Pelvic inflammatory disease can damage or narrow the tubes and is one possible risk factor. A pregnancy may become visible on a repeat scan as it develops, while changing symptoms or examination findings may require faster action.

Contact the maternity or gynaecology team that arranged your assessment and follow the planned schedule for repeat pregnancy-hormone blood tests and imaging. A transvaginal ultrasound may give a clearer view of the uterus, tubes, and surrounding pelvis than an abdominal scan, although an early scan can still be inconclusive.

Keep a record of bleeding, pain location, pain severity, dizziness, and any change in symptoms. Do not wait for a scheduled appointment if symptoms become severe or you feel faint. Until the diagnosis is clear, make sure the clinical team knows about any previous ectopic pregnancy, tubal surgery, fertility treatment, or pelvic infection.

Seek emergency care for sudden or severe pelvic or abdominal pain, pain at the tip of the shoulder, heavy vaginal bleeding, marked weakness, dizziness, fainting, or trouble breathing. These symptoms can signal internal bleeding from a ruptured ectopic pregnancy and need immediate assessment, even if an earlier scan was reassuring or did not show a pregnancy.

A pelvic pain assessment may include an examination, pregnancy testing, blood tests, and repeat imaging. Treatment depends on the pregnancy's location, size and progression, your symptoms, and your overall health; options can include monitored follow-up, medication, or surgery.
