Nighttime wheezing: what should you know?

Short answer
Nighttime wheezing is a whistling breath caused by narrowed airways and may need assessment if it recurs or affects sleep.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Sound
- A high-pitched whistle can occur when air moves through narrowed airways.
- Emergency clue
- Wheeze with throat swelling or severe breathlessness needs urgent help.
What may cause it
Airway narrowing can become more noticeable at night when inflammation, mucus, cold air, or bedroom allergens affect breathing. Asthma commonly causes wheeze with cough, chest tightness, or breathlessness, while allergic rhinitis can add nasal blockage. A chest infection such as acute bronchitis may bring cough and mucus. COPD can also cause recurring wheeze, particularly in people with a smoking or occupational exposure history. In young children, bronchiolitis is another possible cause.
What to do
Sit upright, move away from smoke or a suspected trigger, and follow your personal prescribed inhaler plan. Do not use another person’s inhaler. Keep a record of when wheeze occurs, cough, exposures, activity, and any reliever use. Arrange a clinical review for repeated nighttime symptoms, waking breathless, reduced exercise tolerance, or a cough that does not settle. A clinician may check breathing and review asthma or lung treatment.
When to seek urgent care
Call emergency services for severe breathlessness, blue or grey lips, inability to speak in full sentences, collapse, confusion, or rapidly worsening symptoms. Treat sudden wheeze with facial or throat swelling, widespread hives, or dizziness as a possible anaphylaxis emergency. If a prescribed reliever is not helping as directed, seek urgent medical advice rather than repeatedly delaying care.
Questions for your doctor
Ask whether asthma, allergy, infection, or another lung condition best fits your symptoms. Ask how to use your inhaler, whether you need a written action plan, and what nighttime changes require urgent help.
