What should you know about NICE croup guidance?

Croup affects the voice box and nearby airway, producing a harsh barking cough, hoarse voice, and sometimes a high-pitched noise when breathing in. Fever and runny-nose symptoms may occur, often alongside a common cold. Symptoms can sound worse at night or when a child becomes upset. The noise and breathing effort matter more than how loud the cough sounds, so watch the child’s colour, alertness, and ability to drink.

Keep the child calm and hold them comfortably upright while they are awake. Offer regular fluids and avoid smoke. Do not use steam or force a distressed child to lie down. Record when noisy breathing occurs and whether it is present at rest. A child health consultation can assess severity; a clinician may prescribe medicine to reduce airway swelling. Nebuliser treatment is reserved for particular urgent situations rather than routine home use.

Seek urgent help if noisy breathing is present while the child is resting, the chest or neck pulls in with breaths, the child cannot speak or drink normally, or they become pale, blue, unusually sleepy, or difficult to settle. Drooling with difficulty swallowing needs urgent assessment. Call emergency services for severe breathing difficulty or reduced responsiveness.
