NANDA nursing diagnosis for sepsis: what it means

NANDA terminology helps nurses label responses such as ineffective breathing, reduced fluid balance, confusion, weakness, or impaired tissue perfusion. It is not a replacement for the medical diagnosis of Sepsis and does not identify a single test result. Nurses use observations, vital signs, urine output, mental state, skin findings, and response to treatment to build and update a care plan.

Sepsis may begin with an infection such as Cellulitis, Pneumonia, or a Urinary Tract Infection. Fever, Chills, Feeling Generally Unwell, and Generalised Weakness can occur, but symptoms vary and need clinical assessment.

In hospital, nurses regularly reassess breathing, circulation, alertness, temperature, pain, fluid balance, and the appearance of the skin. They may provide oxygen or fluids when prescribed, give antimicrobial treatment and other medicines on schedule, collect samples, and report changes quickly. Relatives can help by sharing the person’s medicines, allergies, recent infections, and any sudden change in behaviour.

Sepsis is an emergency concern. Seek urgent medical help for a person with a suspected infection who becomes confused, unusually sleepy, breathless, faint, cold and clammy, or rapidly weaker. Do not wait for every symptom or try to manage suspected sepsis at home; worsening illness needs immediate assessment.

Ask which nursing problems are being monitored, what changes should be reported, where the infection may be, how treatment is working, and what recovery support will be needed after discharge.
