My scalp is itchy and oily: what should I know?

Reading the scalp pattern

Greasy scale and itch around the scalp, hairline, ears, or eyebrows can fit dandruff or seborrheic dermatitis. Small tender or itchy bumps suggest folliculitis. Thick, well-defined scale may occur with scalp psoriasis. Head lice cause itching and nits attached close to the hair shaft. Heavy oils, dry shampoo, fragrance, or styling residue may trap debris and irritate the skin, while a new dye or product can trigger contact irritation.

Oiliness alone does not identify the cause. A clinician can examine the scale, follicles, hair shafts, and skin beyond the scalp, including the ears and hairline.

Helpful scalp care

Wash regularly with lukewarm water and rinse thoroughly, choosing a gentle product while the scalp is irritated. Keep oils and styling products off the skin until the cause is clearer, and avoid picking bumps or scraping scale. If medicated shampoo is advised, use it exactly as directed, including its contact time. Do not share brushes, combs, hats, or towels while lice are possible. Note product changes, sweating, flare timing, and whether flakes are loose or stuck.

When to seek medical advice

Arrange an appointment if itch persists, recurs, causes sores, or does not improve with gentle care. Seek prompt help for pus, painful swelling, spreading redness, fever, thick crusting, or new hair-loss patches. Emergency care is needed for facial swelling, breathing difficulty, or a widespread rapidly developing reaction.
