Why tonsils swell

In glandular fever, the tonsils may become enlarged and red, sometimes with a coating. The illness can also cause fever, sore throat, swollen neck glands, fatigue, and a general feeling of being unwell. The appearance of the tonsils does not identify the cause on its own: viral illnesses, strep throat, and tonsillitis can produce similar findings.

A clinician may check your throat, neck, hydration, and other symptoms. Testing may be considered when the diagnosis is uncertain, symptoms are severe, or recovery is not following the expected pattern.

How to care for your throat

Sip fluids often and choose cool or soft foods if swallowing hurts. Rest, avoid smoke, and use suitable pain or fever relief as directed for you. Do not scrape coating from the tonsils. If glandular fever is suspected, avoid strenuous exercise and contact sports until a clinician confirms a safe return.

When to seek care

Get urgent help if swollen tonsils make breathing difficult, you cannot swallow saliva or fluids, your voice becomes muffled, or swelling is rapidly worsening. Seek prompt assessment for severe one-sided throat pain, dehydration, confusion, fainting, or sudden severe upper-abdominal pain.
