Miscarriage without bleeding symptoms

Some pregnancy losses are identified because an ultrasound shows that development or heart activity has stopped before bleeding begins. Pregnancy symptoms may also change, although symptoms alone cannot diagnose a miscarriage. A Transvaginal Ultrasound and, in some situations, repeat blood tests help distinguish an early loss from a pregnancy that is simply earlier than expected.

Contact your maternity or gynaecology service if you are worried about a change in symptoms or have been told a scan is uncertain. Keep any scheduled review, record pain or discharge, and ask which medicines are suitable. If loss is confirmed, your clinician may discuss waiting, Medical Management of Miscarriage, or Surgical Management of Miscarriage and explain follow-up.

Seek urgent help for severe or one-sided pain, shoulder-tip pain, faintness, dizziness, fever, chills, or feeling suddenly unwell. An ectopic pregnancy can present without typical bleeding, and urgent assessment is needed when these warning signs occur.

Ask what the scan shows, whether dates could be uncertain, whether repeat imaging or blood tests are needed, what symptoms should prompt emergency care, and how completion of a miscarriage will be confirmed.
