What should you know about miscarriage twins?

Bleeding or cramping can occur in a twin pregnancy without meaning that both pregnancies have been lost. An ultrasound may show whether one pregnancy is developing, whether both are affected, and whether pregnancy tissue remains. The result depends on the timing of the scan and what can be seen.

Losing one twin while the other continues can bring mixed emotions and may require follow-up tailored to the ongoing pregnancy. If the miscarriage affects the whole pregnancy, a clinician can explain physical recovery and available care. Feelings of grief, worry, confusion, or guilt deserve support.

Contact your maternity or gynaecology service after bleeding, fluid leakage, cramps, or passage of tissue. Record symptoms and bring details of any earlier scan if available. A transvaginal ultrasound may be used to clarify the pregnancy findings.

After assessment, discuss whether observation, medical management, or surgical management is appropriate. Ask how the ongoing pregnancy, if present, will be monitored and which symptoms should lead to another review. Arrange emotional and practical support while decisions are being made.

Seek urgent care for heavy bleeding, severe or one-sided pain, fainting, marked dizziness, shoulder-tip pain, fever, chills, or feeling very unwell. Contact emergency services if you cannot stay safe because of emotional distress. Urgency is based on symptoms, not on whether one or both pregnancies may be affected.

Ask what the ultrasound shows for each pregnancy, whether another scan is needed, what bleeding is expected, and how the pregnancy will be followed if one twin continues. Ask which management options are suitable, how pain will be treated, and where to obtain bereavement support.
