What should you know about miscarriage cramping?

Cramping may be felt low in the abdomen, pelvis, or lower back. It can come in waves as the uterus contracts, and some people pass clots or pregnancy tissue. The intensity and pattern vary, so cramping alone cannot confirm whether a miscarriage is happening. Mild cramps and spotting can also occur in early pregnancy for reasons that are not miscarriage, while an ectopic pregnancy and other urgent problems can cause pain or bleeding. A clinician may ask about your last menstrual period, symptoms, and medical history, then offer an examination, blood tests, or a transvaginal ultrasound to assess the pregnancy and check for other causes.

If a miscarriage is confirmed, care may involve waiting for the pregnancy tissue to pass, medicines that help the uterus empty, or a procedure. The suitable option depends on bleeding, pain, scan findings, your health, and your preferences.

Contact a gynaecology service, maternity unit, or emergency service for advice if you have cramping or bleeding during pregnancy. Note when the pain started, whether it is constant or comes in waves, how much you are bleeding, and whether you have passed tissue. Rest in a position that feels comfortable, drink fluids if you can, and use sanitary pads so bleeding is easier to observe. Do not put anything in the vagina while you are bleeding unless a clinician advises it. Ask before taking pain medicine, particularly if you may still be pregnant or have another medical condition.

A clinician may recommend a transvaginal ultrasound when the pregnancy location or development needs checking. If miscarriage is diagnosed, you can discuss medical management or surgical management, including what to expect, follow-up, and signs of infection.

Seek emergency care for severe or worsening abdominal pain, pain mainly on one side, shoulder-tip pain, fainting, marked dizziness, confusion, or feeling very weak. These symptoms can signal internal bleeding or another emergency. Get urgent help as well for heavy vaginal bleeding, fever, chills, foul-smelling discharge, or pain that is not controlled with the advice you were given. If you are Rh-negative, ask the treating team promptly whether you need anti-D treatment. Do not drive yourself if you feel faint or unsafe.
