Miscarriage after a strong heartbeat at 7 weeks: what to know

At 7 weeks, seeing cardiac activity indicates development was visible at the time of the ultrasound. It does not rule out every later complication. Spotting or mild cramps may occur for different reasons; heavier bleeding, persistent cramps, or a later scan without cardiac activity can be associated with miscarriage, but symptoms do not diagnose it.

A clinician may arrange a repeat transvaginal ultrasound and, when useful, pregnancy hormone testing. The earlier report, measurements, dates, and new findings are considered together rather than relying on one symptom or one home test.

Contact your maternity, fertility, or early pregnancy team and mention that cardiac activity was previously seen at 7 weeks. Describe the colour and amount of bleeding, clots, pain location, dizziness, and any fever. Use pads while bleeding and keep your scan report available for the appointment.

If a miscarriage is confirmed, ask about waiting for the pregnancy tissue to pass, medicines, or a procedure. Each option has different practical considerations, and the recommendation depends on your symptoms, ultrasound findings, medical history, and preference.

Seek emergency care for severe or one-sided pain, shoulder-tip pain, fainting, fever, feeling increasingly unwell, or rapidly heavy bleeding. These warning signs need immediate assessment for ectopic pregnancy, infection, or significant blood loss, even after a reassuring earlier scan.
