What is happening in the shoulder

The subacromial and subdeltoid bursa is a thin, fluid-containing sac between the shoulder tendons, upper arm bone, and nearby tissues. It helps these structures glide. Irritation can make reaching overhead, lying on the affected side, dressing, or lifting uncomfortable. Minimal imaging changes can be one part of the picture and should be interpreted alongside the examination.

Bursitis may occur with repetitive loading, a shoulder tendon problem, arthritis, a direct blow, or less commonly infection. Bursitis presents with shoulder pain and sometimes local swelling; shoulder pain alone does not establish bursitis as the cause.

Practical first steps

Temporarily reduce movements that repeatedly provoke the shoulder, while keeping gentle pain-limited motion so the joint does not become stiff. A physiotherapist can guide range-of-motion and rotator-cuff exercises, then progress resistance as symptoms settle. Ice or another comfortable cooling method may help after activity.

Medication may be considered after checking your medical history, stomach and kidney health, pregnancy status, and other medicines. If symptoms persist, a clinician may discuss an injection or, in selected cases, fluid sampling. Do not inject or aspirate a shoulder without a trained professional.

Seek prompt assessment for a hot, red, markedly swollen shoulder, fever, feeling unwell, or rapidly worsening pain because infection needs urgent consideration. Arrange review for persistent night pain, increasing weakness, inability to lift the arm, or symptoms after a significant injury. Sudden chest pain or breathing difficulty requires emergency help rather than shoulder treatment.
