Where the irritation occurs

Intermetatarsal bursae sit between the long bones of the forefoot. Repeated pressure from narrow footwear, prolonged standing, altered walking, or a change in activity can irritate one of these sacs. Symptoms may include tenderness in the ball of the foot, a feeling of fullness, or pain when pushing off. Joint swelling in another area or persistent forefoot swelling can point to a different problem and should be assessed.

Ways to reduce pressure

Choose footwear with enough room for the toes and a supportive sole. Temporarily reduce impact and activities that reproduce forefoot pain, then return gradually as walking becomes easier. A clinician may suggest padding, footwear changes, or exercises that address calf and foot mechanics. Anti-inflammatory medication may be appropriate for some people after checking medical history. Cortisone injection is a clinician-led option when symptoms persist, while joint aspiration is considered only when fluid needs evaluation or removal.

When to seek medical care

Arrange assessment if pain persists despite pressure reduction, swelling increases, footwear becomes difficult to tolerate, or numbness develops between the toes. Seek prompt care for marked redness, warmth, fever, an open wound, or rapidly worsening pain, especially if you have diabetes or reduced sensation. A clinician can check for nerve irritation, infection, or another forefoot cause.
