Why fatigue needs context

MGUS is a blood finding involving a monoclonal protein. Fatigue is not specific to MGUS, and the finding may be unrelated to how tired you feel. Sleep problems, stress, low mood, medicines, infection, dehydration and reduced activity can all affect energy.

A clinician may also consider anaemia, kidney disease, coeliac disease or another condition when the history or examination points that way. Chronic fatigue syndrome and burnout have different diagnostic features, so labels should not be applied from tiredness alone.

How to prepare for assessment

Record when fatigue began, whether it is constant, what activity does to it, and whether rest helps. Note sleep quality, appetite, weight change, pain, fever, breathlessness, thirst, urine changes and mood. Bring your medicine list and any previous MGUS reports.

Maintain regular fluids, balanced meals and gentle activity if these are comfortable. Avoid using stimulants to push through severe exhaustion without discussing the pattern with a clinician. Follow the monitoring plan for MGUS even if fatigue improves.

When to seek care

Arrange prompt medical review if fatigue is worsening, limits ordinary activities, or occurs with unexplained weight loss, persistent fever, night sweats, bone pain, repeated infections, swelling or changes in urination. Seek emergency help for severe breathlessness, collapse, new confusion or chest pain.
