What it may mean

A hot flush, anxiety, poor sleep or reduced fitness can make breathing feel harder, but breathlessness has many possible explanations. Notice whether it occurs at rest, during exertion, at night, with wheeze, or with a racing or irregular heartbeat. Anaemia can reduce oxygen delivery; asthma, COPD, bronchiectasis and acute bronchitis can affect the airways. Angina may cause breathlessness with chest pressure, while atrial fibrillation can produce palpitations and reduced exercise tolerance. Anaphylaxis can develop rapidly with swelling or breathing difficulty. The timing and associated symptoms matter more than assuming menopause is the cause.

What you can do

Stop and rest when breathlessness starts, loosen tight clothing and note the activity, duration, temperature, symptoms and recovery. Avoid smoke and other known irritants, and do not use another person’s inhaler. Arrange a medical assessment for new, recurring or worsening breathlessness; a clinician may listen to your chest, check pulse and oxygen level, and decide whether blood tests or heart and lung checks are needed. Mention hot flushes, panic sensations, sleep disruption, medicines and any history of asthma or heart disease.

When to seek care

Call emergency services for severe or sudden breathing difficulty, blue or grey lips, confusion, fainting, inability to speak normally, chest pressure, or swelling of the face, tongue or throat. Get urgent help if breathlessness follows a food, medicine or insect exposure, or if it comes with coughing blood or a very fast, irregular heartbeat. Do not drive yourself if symptoms are severe.
