Lump on Neck That May Not Be a Lymph Node

The neck contains the thyroid gland, salivary glands, muscles, skin, blood vessels, nerves and lymph nodes. A lump may be a cyst, an inflamed skin structure, a thyroid nodule, a muscle-related swelling or another mass. The way it feels by touch cannot establish the cause. A neck lump can be a presentation of head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary, but many neck lumps have non-cancerous explanations.

A clinician will consider its position, size, firmness, mobility, tenderness and how long it has been present. Examination of the mouth, throat, ears and thyroid may help decide whether imaging or a needle sample is appropriate.

Arrange a clinical assessment, especially if the lump is new, enlarging, firm or fixed. Note when you first noticed it, whether it changes with swallowing or movement, and whether infection, injury or dental symptoms came first. Do not repeatedly squeeze or massage it, because this can irritate the tissue and make changes harder to judge.

The clinician may recommend an ultrasound or other scan. If tissue is needed, a fine-needle sample can help distinguish thyroid, inflammatory and other cell types. Treatment depends on the confirmed cause; thyroid-related findings may lead to a discussion of Thyroid Removal Surgery when surgery is appropriate.

Seek urgent medical help for trouble breathing, inability to swallow fluids, rapidly increasing swelling, or a change in voice with breathing difficulty. Request a prompt appointment for a persistent or growing lump, unexplained weight loss, night sweats, coughing blood, persistent sore throat, ear pain on one side, or difficulty swallowing. These signs do not diagnose cancer, but they should not be assessed by watchful waiting alone.

Ask: Which structure does the lump seem to involve? Do I need an ultrasound, scan or needle sample? What changes should I monitor? If the thyroid is involved, what do my thyroid tests and imaging show? When should I return if the lump remains?
