What should you know about lump on neck below ear?

A lump below the ear can arise from a lymph node, salivary gland, skin, or nearby infection, and its persistence or change should guide medical review. The location gives a clinician a useful starting point, but it cannot identify the cause by itself.

Below the ear are lymph nodes and part of the parotid salivary-gland region, along with skin and soft tissue. A node may enlarge with an ear, throat, dental, scalp, or skin infection. Salivary-gland swelling may feel fuller around meals or with dry mouth, while a skin cyst may be close to the surface. A firm, fixed, painless, or enlarging neck mass requires assessment. Head and neck cancer, lymphoma, and thyroid cancer can present with neck swelling, but these conditions cannot be diagnosed from the position of a lump. When no starting site is immediately clear, clinicians investigate the mass systematically.

The examination may include the ears, mouth, teeth, throat, skin, salivary glands, and thyroid area. Imaging or tissue sampling is chosen when the findings call for it.

Record when the swelling began and whether eating, swallowing, or touching changes it. Note pain, fever, dry mouth, ear symptoms, dental trouble, skin redness, or discharge. Avoid squeezing or attempting to puncture it. Make an appointment when the lump persists, grows, feels firm or fixed, or lacks a clear short-lived explanation. Bring information about recent infections, dental work, medicines, and any previous neck problems.

Seek urgent help for breathing difficulty, inability to swallow saliva, rapidly increasing swelling, or severe illness. Arrange prompt assessment for a hard or fixed lump, progressive enlargement, persistent hoarseness, significant swallowing pain, facial weakness, or swelling that makes mouth opening difficult. These features can involve structures near the airway, nerves, salivary glands, or throat and need direct examination.

Ask whether the swelling is in a lymph node, salivary gland, or skin. Could ear, dental, throat, or scalp symptoms explain it? Would an ultrasound show its location and structure? If a sample is needed, ask what it can distinguish and how soon you should receive follow-up. Confirm which changes require earlier contact.
