Lump behind the head above the neck: what to know

The junction between the back of the head and upper neck includes scalp, connective tissue, muscle, and lymph nodes that drain nearby areas. A scalp sore, insect bite, skin inflammation, or ear or throat infection can cause a nearby node to swell. Cysts and other benign lumps may also occur in the skin or soft tissue.

A lump in the neck can be a presentation of lymphoma or head and neck cancer, but the same symptom also occurs with many non-cancerous conditions. Size, firmness, mobility, tenderness, duration, and associated symptoms are more useful than the location alone when planning an assessment.

Look for a nearby scalp or skin problem without picking at it. Note when the lump began, whether it changes, and whether you have had an infection or injury. Do not squeeze or puncture the area. Arrange a clinical examination if the lump remains, grows, feels hard or fixed, or appears without a clear short-term trigger. The clinician may inspect the scalp and ears and examine the full neck before selecting any test.

Contact a clinician promptly for a persistent or enlarging lump, unexplained weight loss, drenching night sweats, or ongoing fever. Seek urgent help if the swelling is associated with breathing or swallowing difficulty, severe pain, rapidly spreading redness, or sudden marked enlargement. Do not use the presence of a lump alone to predict cancer.

Ask whether the lump appears to be a lymph node, skin lesion, or deeper mass; whether the scalp and ears show a possible trigger; what examination or imaging is needed; and how soon it should be reviewed if it changes.
