What should you know about a lump at the top of the neck?

Why the upper neck matters

The upper neck contains lymph nodes, salivary glands, muscles, and tissues close to the ear, jaw, throat, and scalp. Dental, throat, ear, or skin infections can make nearby lymph nodes swell. A salivary-gland or skin problem may feel close to the jaw or under the ear. Some head and neck cancers and lymphomas present with neck nodes, yet a lump at the top of the neck does not identify one of these conditions. A clinician uses the exact position, texture, movement, and associated symptoms to decide what to investigate.

Planning an assessment

Contact a clinician if the lump is new and does not settle, grows, feels firm or fixed, or returns after improving. Note dental pain, sore throat, ear symptoms, scalp changes, fever, swallowing difficulty, voice change, and the date you first noticed the swelling. The examination may include the mouth, teeth, throat, ears, salivary glands, and lymph-node areas. An ultrasound, other imaging, or a needle sample may follow when the findings warrant it. Do not squeeze a lump near the jaw or ear.

Urgent and timely warning signs

Seek urgent help for trouble breathing, inability to swallow saliva, rapidly increasing swelling, or severe pain with fever. Arrange timely review for a persistent, hard, fixed, or enlarging lump, especially with unexplained weight loss, drenching night sweats, coughing blood, or ongoing hoarseness. These signs indicate a need for appropriate assessment; they do not mean that the lump has a particular diagnosis.

What to ask

Ask which nearby structure seems involved, whether dental or ear findings could explain the swelling, and whether imaging is needed. Ask when the lump should be rechecked, how results will be communicated, and which new symptoms should change the urgency.

Yes. Nearby infections can activate lymph nodes, often with tenderness or other symptoms in the mouth, throat, ear, or skin.

Is a firm upper-neck lump automatically cancer?

No. Firmness is one feature clinicians consider alongside duration, movement, growth, and other symptoms. Persistent or changing lumps should be examined.
