Short answer

Leaking urine while running often reflects stress urinary incontinence, which can improve with pelvic floor assessment and tailored treatment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common trigger
Impact and increased abdominal pressure during running
Useful record
Timing, activity, fluids, urgency, and urinary symptoms
First assessment
History, examination, and sometimes a urine test

What it can mean

Running repeatedly increases pressure inside the abdomen. If the pelvic floor muscles and the bladder outlet do not close firmly at that moment, urine may escape with impact, sprinting, jumping, or coughing. A small leak can still be worth discussing, especially if it is new, becoming more frequent, or affecting exercise choices. Urgency, burning, cloudy urine, pelvic pain, or fever may point to another problem such as a urinary tract infection. In men, urinary symptoms can also occur alongside benign prostatic hyperplasia. Diabetes and higher body weight can add pressure or affect bladder function.

What to do next

Keep a brief record of when leakage happens, how much you notice, what you drank, and whether urgency or burning is present. Emptying your bladder before a run may help, but avoid repeatedly restricting fluids because concentrated urine can irritate the bladder. A clinician or pelvic health physiotherapist can check pelvic floor technique and build a gradual strengthening plan. Consider lower-impact training while symptoms are assessed, and use a suitable pad if needed. Do not practise stopping urine midstream as a regular exercise.

When to seek care

Arrange a routine appointment if leakage persists, limits running, or occurs with urgency, night-time urination, weak flow, or incomplete emptying. Seek prompt medical advice for blood in the urine, fever, pain in the side or back, painful urination, or sudden difficulty passing urine. New loss of bladder control with leg weakness, numbness around the groin, or severe back pain needs urgent assessment.

Questions for your doctor

Ask whether the pattern fits stress, urge, or mixed urinary incontinence; whether a urine test is needed; how to perform pelvic floor exercises correctly; and whether your training, medicines, bowel habits, or prostate symptoms could be contributing.
