What the swelling may represent

A bursa reduces friction where skin, tendons, ligaments, or bone move against one another. Repeated rubbing from footwear, pressure, altered walking, or an ankle injury can irritate this sac. The outer ankle may look puffy and feel sore when touched or when shoes press on it. Warmth and redness can occur, but they also raise the need to consider infection or another diagnosis.

A clinician may examine the exact location, skin, ankle movement, gait, and nearby tendons. The term bursitis should be confirmed rather than assumed, because tendon irritation, a sprain, gout, or infection can cause similar symptoms.

Ways to settle irritation

Reduce activities and footwear that repeatedly press on the tender area. Rest with the ankle supported, use a cold pack wrapped in cloth for short periods, and return to walking gradually as discomfort allows. Anti-inflammatory medication may be suitable for some people, but it can interact with other medicines or health conditions, so check with a clinician or pharmacist.

If symptoms persist, a clinician may discuss a cortisone injection. Fluid removal, called joint aspiration, is considered only when it fits the suspected cause and may allow fluid to be examined. Do not puncture or drain the swelling yourself.

When to seek medical care

Seek prompt medical assessment if the outer ankle becomes rapidly more swollen, hot, red, or very painful, or if you develop fever, chills, or feel unwell. Arrange review after an injury if you cannot bear weight, the ankle looks deformed, or pain is worsening. Persistent swelling, drainage, or a lump that does not settle also needs examination.
