What should you know about lactose allergy tests?

A lactose allergy test is not usually the right test for lactose intolerance, because allergy and intolerance involve different immune and digestive processes.

Allergy and intolerance are different

Lactose is the natural sugar in milk. Lactose intolerance happens when the small intestine has too little lactase, the enzyme that breaks lactose down. Undigested lactose can draw water into the bowel and be fermented by gut bacteria, causing gas, cramps or loose stools.

A milk allergy is an immune reaction to milk proteins, not to lactose. It may cause hives, swelling, vomiting, wheezing or breathing difficulty soon after exposure. Skin-prick or blood allergy testing may help assess suspected food allergy, while a clinician may use symptom timing, dietary changes and sometimes a supervised lactose challenge to assess intolerance.

How to prepare for assessment

Keep a brief diary of foods, portions, symptoms and how soon they begin. Do not remove all dairy for a long period without advice, especially if you are pregnant, breastfeeding, growing, or have another condition affecting nutrition. A clinician can help distinguish milk allergy from lactose intolerance and suggest suitable calcium and protein sources.

When symptoms need prompt attention

Seek emergency help for trouble breathing, throat or tongue swelling, faintness, or rapidly spreading hives after milk exposure; these can be signs of anaphylaxis. Arrange medical review for recurring diarrhoea, weight loss, blood in the stool, persistent vomiting, or symptoms that disrupt eating and daily activities.
