What should you know about knee hot and cold therapy?

How heat and cold differ

Cold can reduce the sensation of pain and help limit swelling after a flare or recent activity. Heat can make stiff muscles and joints feel more comfortable before gentle movement. Neither treatment repairs a torn structure or explains why the knee is swollen. Similar symptoms can occur with an ankle sprain, muscle contusion, muscle strain, Osgood-Schlatter Disease, joint swelling, or muscle soreness after exercise, so the surrounding injury and timing matter.

Using packs safely

Place a cold or warm pack over a cloth rather than directly on skin, and check the skin regularly. Use cold after activity when swelling is the main issue; try gentle warmth before movement when stiffness or muscle tightness is the main issue. Stop if the skin becomes painful, very pale, numb, or intensely red. Do not fall asleep with a pack in place, and avoid heat over a newly swollen area or an area with reduced sensation. When home measures are not enough

Arrange assessment if knee swelling persists, pain keeps returning, movement becomes limited, or the knee gives way or locks. Seek urgent care after an injury if you cannot bear weight, the knee is deformed, swelling develops rapidly, or the joint is hot with fever or you feel unwell. Stop self-treatment and seek advice if heat or cold repeatedly worsens symptoms.

Tell the clinician when symptoms began, what caused them, where swelling sits, whether the skin feels normal, and which activities increase discomfort.
