Keloid on the buttocks: what to know

Keloids are raised scars that extend beyond the original injury, such as acne, an injection, an operation or an irritated hair follicle. On the buttocks they may feel firm, itch when clothing rubs them or become sore with sitting. Other causes of a lump include an inflamed follicle or cyst. A clinician can distinguish these by examining the surface, depth and borders.

Keep the area clean, choose breathable clothing and reduce rubbing during exercise or prolonged sitting. Do not squeeze, puncture or repeatedly scrub the lesion. Depending on the diagnosis, a dermatologist may discuss steroid injection, cryotherapy, laser treatment or excision. Excision may be followed by additional scar care because removing a keloid can itself create a new wound. Seek prompt care for rapid enlargement, severe pain, warmth, pus, spreading redness or fever. Make a routine appointment for a persistent, itchy or changing lump, or one that makes sitting difficult. Urgent help is needed if you feel faint, severely unwell or the swelling becomes suddenly extensive.

Ask what caused the scar, whether it is safe to observe, and which treatment best fits its depth and location. Discuss ways to reduce friction, expected healing and how recurrence would be monitored.
