What should you know about keloid microneedling?

Microneedling uses fine needles to make many controlled punctures in the skin. The healing response can support texture improvement for selected concerns, but any deliberate skin injury may encourage excessive scar tissue in someone who forms keloids. A keloid is a raised, firm scar that can extend beyond the original wound and may continue enlarging or feel itchy, tender, or tight.

A previous keloid after an injection, piercing, operation, acne lesion, or minor cut is relevant history. So is a family tendency to keloids. Treating directly over a keloid is different from treating an unrelated area, yet both situations need careful review because new punctures can create additional scars. Home rollers and unsterile devices add risks such as infection and uneven injury.

Tell the clinician about every raised scar you have had, where it formed, whether it kept growing, and any treatment used for it. Bring photographs if an older scar has flattened or is not easy to see. Do not start microneedling on a keloid or keloid-prone area without a personalised plan from a qualified dermatologist.

The clinician may recommend avoiding needling, treating a different concern with a different approach, or managing an existing keloid first. Ask how the device will be sterilised, which areas will be avoided, what early scar changes should be reported, and how follow-up will work. Options such as standard microneedling, a microneedling pen, RF microneedling, or fractional laser treatment are not interchangeable; each can affect skin differently and still needs suitability screening.

Contact the treating clinic promptly if a treated spot becomes increasingly raised, firm, itchy, painful, tight, or extends beyond the treated marks during healing. Seek urgent medical help for rapidly spreading redness, worsening swelling, pus, fever, or severe pain, which can signal infection or another complication. A dermatologist can distinguish expected irritation from a developing scar and decide whether early treatment is needed.
