Itchy scalp but no lice: other causes

Dandruff and seborrheic dermatitis can cause itch with white or greasy scale. Scalp psoriasis may produce thicker attached areas of scale. Folliculitis is more likely to cause small itchy or tender bumps around hairs. A new shampoo, dye, oil, or styling product can irritate the skin, while sweat, friction, and frequent washing may add to discomfort. A negative lice check does not point to one replacement diagnosis; the scalp pattern still needs to be considered.

Check close to the roots in bright light, since loose debris can be mistaken for nits. If uncertainty remains, a professional examination is more reliable than repeated treatment.

Stop suspected new products and wash with a gentle cleanser, using lukewarm water and a thorough rinse. Avoid scratching, tight headwear, and sharing combs until lice concerns are settled. Do not repeat lice medicine simply because itching continues; itch can persist from irritation or another condition. If scale is present, ask about a medicated shampoo and follow its directions carefully.

Arrange a review if itch continues after lice have been reasonably excluded, keeps returning, affects sleep, or is joined by bumps, sores, crusting, pus, pain, or hair loss. Seek prompt care for fever, rapidly spreading redness, or swelling of the face. Bring details of the lice check, products tried, and any visible changes.
