Itchy scalp without dandruff: possible causes

Not every scalp problem produces loose flakes. Contact irritation may cause itch, burning, or redness soon after a new shampoo, dye, oil, or styling product. Folliculitis tends to produce small tender or itchy bumps around hairs. Scalp psoriasis can form thicker plaques that cling to the scalp rather than falling as dandruff. Lice may cause intense itch with little visible scale; inspect near the roots for live insects or firmly attached nits.

The absence of dandruff does not identify a single cause. The pattern, timing, and appearance of the skin matter more than the label used for the symptom.

Review recent changes in hair products, washing frequency, hats, helmets, and chemical treatments. Pause suspected irritants, use a mild fragrance-free cleanser, rinse completely, and avoid hot water and vigorous scratching. Do not start repeated lice treatment unless lice or nits have been identified. If bumps or redness remain, a clinician or pharmacist can guide treatment rather than relying on trial and error.

Seek an assessment when itch lasts, returns often, affects sleep, or comes with pain, pus, crusts, sores, swelling, or patchy hair loss. Prompt care is appropriate for fever, rapidly spreading redness, or facial swelling. Take photographs of intermittent changes and bring the products used on the scalp.
