Short answer

An itchy and sore scalp can result from irritation, dandruff, inflammation, infection, or head lice, so the appearance of the scalp and accompanying symptoms help guide the next step.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common clues
Flakes, plaques, tender bumps, crusting, or visible nits can point to different causes.
First step
Stop new hair products, wash gently, and avoid scratching the sore skin.
Prompt review
Seek care for pus, spreading redness, fever, severe pain, swelling, or patchy hair loss.

What an itchy and sore scalp may mean

Fine flakes and itch may occur with dandruff or seborrheic dermatitis, which can also cause greasy scale and irritation around the hairline, ears, or eyebrows. Thick, sharply defined plaques that extend beyond the hairline can occur with scalp psoriasis.

Tender bumps, pustules, or crusting around individual hairs may point to folliculitis. Widespread itch with visible nits or crawling insects suggests head lice, especially when other household members are affected. A new hair product, dye, shampoo, or styling chemical can trigger contact irritation or an allergic reaction.

Itchy scalp is a symptom rather than a diagnosis. Scratching can break the skin and increase soreness, bleeding, or the chance of a secondary infection. Hair loss in patches, marked swelling, or a painful area deserves a clinician's assessment.

What you can do now

Pause any recently introduced hair product and rinse the scalp gently with lukewarm water. Avoid picking or scratching, tight hairstyles, and vigorous brushing over sore areas. Keep combs, brushes, hats, and pillowcases personal until head lice has been excluded.

If flaking is the main problem, a medicated shampoo treatment may be suitable when used exactly as directed; allow the product to remain on the scalp for the stated contact time and stop if it causes marked burning or swelling. Do not share treatment products, and seek advice before using them on a child, during pregnancy, or on broken skin.

A pharmacist or dermatologist can distinguish scale from nits, eczema, psoriasis, or folliculitis by examining the scalp. Treatment may require a different shampoo, an anti-inflammatory product, or therapy directed at an infection or infestation.

When to seek medical care

Arrange a clinical review if soreness persists, the scalp is repeatedly crusting, or symptoms return despite stopping a suspected irritant and following product instructions. Seek prompt care for spreading redness, warmth, pus, significant swelling, fever, or severe pain.

Get assessed for patchy hair loss, broken hairs, swollen glands, or a rash affecting the face or ears. Urgent help is appropriate if swelling of the scalp is accompanied by facial swelling, breathing difficulty, or widespread blistering. Do not delay evaluation when a baby or young child has extensive scalp inflammation.

Questions for your doctor

Ask: What does the scale, rash, or tenderness suggest? Could a product reaction, seborrheic dermatitis, psoriasis, folliculitis, or head lice explain it? Which treatment should touch the scalp, how often should it be used, and when should improvement be expected?

Also ask whether close contacts need checking, which signs would mean the treatment is irritating your skin, and whether patchy hair loss or recurrent symptoms require further tests or a dermatology referral.

Questions people ask

Can dandruff make the scalp sore?

Yes. Flaking and inflammation can make the scalp itchy, tender, or irritated, particularly when scratching damages the skin.

How can you tell head lice from dandruff?

Nits are attached firmly to individual hairs near the scalp, while dandruff flakes usually loosen more easily. A clinician or pharmacist can confirm what you are seeing.

Should you scratch an itchy scalp?

Try not to. Scratching can create small breaks in the skin, increase soreness, and allow bacteria to enter.
