IT band pain near the hip: what to know

Short answer
IT band pain near the hip may reflect irritation of tissues at the outer hip, but an examination is needed to distinguish it from other causes of hip pain.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical location
- Outer hip or outside of the thigh
- Useful clue
- Pain may change with walking, stairs, running, or lying on one side
- Assessment focus
- Hip movement, pelvic control, strength, and nerve signs
What the pain may mean
The iliotibial band is a strong strip of connective tissue running down the outside of the thigh. Repeated hip movement, changes in running or walking, or weakness in the muscles that steady the pelvis can make the outer hip feel sore or tight. Pain can also come from the gluteal tendons, the hip joint, the lower back, or an injury. Gluteal tendinopathy can cause tenderness over the bony outer hip, while hip impingement or a labral tear may cause deeper groin or hip symptoms. Hip osteoarthritis may produce stiffness after rest and pain with movement. A clinician compares the location, triggers, strength, and range of motion rather than relying on the label IT band pain alone.
What you can do
Temporarily reduce the activity that reliably brings on pain, especially repeated hills, long runs, or side-lying on the sore side. Keep gentle, comfortable movement so the hip does not become guarded. A physiotherapist can assess hip and pelvic control, then build a gradual strengthening plan for the gluteal and trunk muscles. Do not force stretches or massage directly over a sharply painful spot. Record when symptoms start, where they travel, and whether stairs, sleeping, or walking change them. More persistent or deep joint symptoms may need medical assessment; total hip replacement is reserved for selected severe joint disease, not unexplained outer-hip pain.
When to seek care
Arrange an assessment if pain lasts despite reducing the provoking activity, keeps returning, limits sleep or walking, or follows a fall. Seek urgent care after a significant injury if you cannot bear weight, the leg looks shortened or rotated, or pain is severe. New weakness, numbness, loss of bladder or bowel control, fever, or a hot swollen joint also needs prompt medical attention. Sciatica can cause pain with nerve symptoms, while a hip fracture requires urgent evaluation after trauma.
Questions for your doctor
Ask which structure appears to be causing the pain, whether you need imaging, and which activities should be modified. Discuss how to progress strengthening, what symptom change should prompt review, and whether your back or hip joint needs assessment.
